Registered nurses lead the assessment, the plan and the review — not only the clinical tasks inside it.

Being nurse-led means clinical oversight is built into how we assess, plan and monitor care — particularly when a person's health or support needs become more complex.

  • Assess

    A registered nurse is part of understanding what someone actually needs, not just what is being asked for.

  • Plan

    Supports are matched to assessed needs, goals and preferences, and written down where everyone involved can see them.

  • Monitor

    Changes in health or circumstances are noticed and acted on, which is what keeps a plan safe as needs become more complex.
